We accept most major PPO, EPO, POS and traditional insurance plans. We are NOT providers for United HealthCare PPO or PacifiCare PPO.

Co-pays and deductibles are due at time of service. We will only accept cash or credit for new patient first time visits. There will be a $25.00 service charge for returned checks.
